Electronic Prescription Service now Available.

We are now able to send repeat and acute prescriptions electronically directly to your nominated pharmacy. This means that you know longer need to come to the surgery to pick up your repeat prescription, you can go directly to the pharmacy and pick it up from there. Please inform reception of your nominated pharmacy. 

Unfortunately we cannot accept requests for repeat prescriptions over the telephone as this could cause errors.

On registering with Upper Halliford Medical Centre all patients are assigned a General Practitioner who is Dr. Kamil. Any queries please ask at the Reception desk.

CQC report findings

The CQC found the practice to be in need of improvement.

 

Repeat Prescriptions

Repeat prescriptions are issued by computer together with a tear-off slip, which is a copy of repeat medication. When requesting a repeat prescription please tick the items required on that copy and post or bring it to the surgery.

If you wish to have your prescription posted please provide a stamped self-addressed envelope.

Alternatively you can order online via the link at the top of this page. New users are required to complete a simple registration prior to using the service.

Pharmacy Services

Alliance and Trio Pharmacies (both in Shepperton) can pick up and deliver prescriptions and medication if requested.

Medication Reviews

Patients on repeat medication will be asked to see a doctor, nurse practitioner or practice nurse at least once a year to review these regular medications and notification should appear on your repeat slip.

Please ensure that you book an appropriate appointment to avoid unnecessary delays to further prescriptions.

Please allow two full working days for prescriptions to be processed and remember to take weekends and bank holidays into account.
